Caption Guidance is an FDA-cleared AI feature that guides clinicians through cardiac ultrasound acquisition with real-time, turn-by-turn probe positioning instructions. Originally developed by Caption Health (acquired by GE HealthCare in 2023), the system displays directional arrows and a quality meter on screen as the user scans. When the image reaches diagnostic quality, Caption Guidance can automatically capture the optimal clip without manual intervention. Built for point-of-care settings where dedicated sonographers may not be available, Caption Guidance enables nurses, hospitalists, and non-specialist physicians to perform limited cardiac assessments with confidence.

About Caption Guidance

Caption Guidance uses deep learning models trained on hundreds of thousands of echocardiographic images to recognize standard cardiac views in real time. The AI continuously analyzes the live B-mode image stream and compares it against learned patterns for the 10 standard echo views (parasternal long axis, parasternal short axis, apical four-chamber, apical two-chamber, apical three-chamber, subcostal four-chamber, subcostal IVC, and others). On screen, the clinician sees directional cues (tilt left, rotate clockwise, slide toward sternum) that guide probe movement toward the target view. A quality gauge fills as the image approaches diagnostic criteria, giving continuous visual feedback. The AutoCapture function stores a clip automatically once image quality crosses the diagnostic threshold, removing the need to press a button at the right moment. Caption Guidance received its initial FDA 510(k) clearance in 2020 and has been validated in multiple clinical studies showing that novice users achieve diagnostic-quality images comparable to those acquired by trained sonographers. On GE Venue systems, it integrates directly into the cardiac exam workflow and works with standard phased array probes.
